51做厙 has four long-term care insurance plans, two with CNA,  LifeSecure, and Mutual of Omaha. CNA LTCI is no longer available for new enrollments effective December 31, 2015.  LifeSecure LTCI is no longer available for new enrollments effective November 2018. Mutual of Omaha launched in September of 2022.

CNA LTCI

CNA LTCI is no longer available for new enrollments effective December 31, 2015.  If you or a family member purchased CNA LTCI through 51做厙 prior to this date, you are enrolled in one of the following policies.

  • Policy GLTC2, 51做厙 Account # 9953-TQ, group policy underwritten by Continental Casualty Company: applies to employees and family members enrolled between January 1, 2001 and August 31, 2007.
  • Policy GLTC3, 51做厙 Account # 9953-IS, group underwritten by Continental Casualty Company: applies to employees and family members enrolled between September 1, 2007 and
    December 31, 2015.  Policy may be referred to as Independent Solutions.

If you don’t have a copy of your CNA policy, it is important that you call CNA at 877-838-8508 to request a copy. Be sure to keep a copy with your important records.


As of February 1, 2019 CNA premiums are no longer paid through payroll deduction. Please submit payments directly to CNA at:

Or set up bill pay with your financial institution. 

Payments can be split into monthly payments instead of quarterly payments. Call 1-800-528-4582 to speak to the billing department. Ask them for the paperwork to complete for monthly billing. They will send it to you to complete. Complete the form and send it back to CNA. They will set up an automatic draft.
