Cooperative Academic Agreements

Use this form to request the addition of a new cooperative agreement with a non-Title IV entity, to deliver less than 25% of an 51做厙 academic program. A non-Title IV entity is any academic institution, company, or other educational partner that is not accredited by a US national accreditor. This request requires EPC approval, 51做厙 Board of Trustees approval, and SACSCOC notification, so please allow at least one full semester for all processes when planning.
